Midazolam drug helps recover full consciousness in traumatic brain injury patient

A patient who had suffered a traumatic brain injury unexpectedly recovered full consciousness after the administration of midazolam, a mild depressant drug of the GABA A agonists family. This resulted in the first recorded case of an "awakening" from a minimally-conscious state (MCS) using this therapy. Although similar awakenings have been reported using other drugs, this dramatic result was unanticipated. It is reported in Restorative Neurology and Neuroscience.

Traumatic brain injuries occur at high rates all over the world, estimated at 150-250 cases per 100,000 population per year. These injuries can result in several outcomes, ranging from vegetative state, minimally conscious state, severe disability to full recovery. In most cases, the outcome will cause catastrophic changes for his/her family and a significant drain on both human and financial resources.

Two years after the injury caused by a motor vehicle accident, the patient was mildly sedated, in order to undergo a CT scan, using midazolam instead of the more commonly used propofol. As the authors described in the article, the patient began to interact with the anesthetist and soon after with his parents. He talked by cellphone with his aunt and congratulated his brother when he was informed of his graduation; he recognized the road leading to his home. When he was asked about his car accident, he did not remember anything and apparently he was not aware of his condition. This clinical status lasted about two hours after drug administration and disappeared quickly thereafter, taking the patient back to the previous condition.

Scientists zero in on obesity pill 'that could replace the treadmill'

Scientists at Harvard University have created a way of screening potential drugs that turn white fat cells – which are "bad" – into the brown fat cells that are healthier. They have already identified two compounds that work on human cells growing in the laboratory.

Researchers believe it may be possible to lower the levels of white fat, which is linked with diabetes and heart disease, by increasing the proportion of brown fat, which burns off excess energy. It could lead to a “pill that can replace the treadmill” for the control of obesity, they said.
Meanwhile, Imperial College researchers in London have identified an enzyme that drives the craving for sugar in the brain’s hypothalamus, which regulates food intake. The scientists believe the enzyme, called glucokinase, could be a viable target for an anti-obesity drug.

“This is the first time anyone has discovered a system in the brain that responds to a specific nutrient, rather than energy intake in general. It suggests that when you’re thinking about diet, you have to think about different nutrients, not just count calories,” said James Gardiner of Imperial College, who led the study published in the Journal of Clinical Investigation.

Tests on rats showed that boosting the activity of the enzyme within the brain caused the animals to consume more glucose in preference to normal food. A drug targeting glucokinase or its metabolic pathway could potentially prevent obesity by lowering the desire for sugary foods, the scientists suggest.

Carboplatin and paclitaxel show promise for advanced thymic carcinoma

A multicentre, phase II study of carboplatin and paclitaxel (CbP) in chemotherapy-naïve patients with advanced thymic carcinoma has shown that the treatment has promising efficacy compared with standard anthracycline-based chemotherapy.

Thymic carcinoma is very rare, and consequently it is hard to investigate it separately from thymoma. Previous studies evaluating chemotherapy regimens have included patients with both types of tumour, explain Takashi Seto (National Kyushu Cancer Center, Fukuoka, Japan) and colleagues.

Forty patients from 21 centres across Japan were enrolled in the current study from May 2008 until November 2010. One patient subsequently dropped out. The sample size was decided on the basis that it was large enough to reject the primary endpoint of an objective response rate (ORR) of 20%.

Injectable 3-D vaccines could fight cancer, infectious diseases



One of the reasons cancer is so deadly is that it can evade attack from the body's immune system, which allows tumors to flourish and spread. Scientists can try to induce the immune system, known as immunotherapy, to go into attack mode to fight cancer and to build long lasting immune resistance to cancer cells. Now, researchers at the Wyss Institute for Biologically Inspired Engineering at Harvard University and Harvard's School of Engineering and Applied Sciences (SEAS) show a non-surgical injection of programmable biomaterial that spontaneously assemblesin vivo into a 3D structure could fight and even help prevent cancer and also infectious disease such as HIV. Their findings are reported in Nature Biotechnology.

Selenium compounds appear to have beneficial effect on cancer

The immune system is designed to remove things not normally found in the body. Cells undergoing change, e.g. precursors of cancer cells, are therefore normally recognised and removed by the immune system. Unfortunately, the different cancer cells contain mechanisms that block the immune system's ability to recognise them, allowing them to freely continue cancer development.

Certain cancer cells overexpress immunostimulatory molecules in liquid form. Such over-stimulation has a negative impact on the immune system:

"You can say that the stimulating molecules over-activate the immune system and cause it to collapse, and we are, of course, interested in blocking this mechanism. We have now shown that certain selenium compounds, which are naturally found in, e.g., garlic and broccoli, effectively block the special immunostimulatory molecule that plays a serious role for aggressive cancers such as melanoma, prostate cancer and certain types of leukaemia," says Professor Søren Skov, Department of Veterinary Disease Biology, University of Copenhagen.

FDA Issues Complete Response Letter for Macrilen (macimorelin) NDA in Adult Growth Hormone Deficiency

Macimorelin is a drug being developed by Æterna Zentaris for use in the diagnosis of adult growth hormone deficiency. As of January 2014, it is in Phase III clinical trials. 
Macimorelin is a mimic of ghrelin, a growth hormone secretagogue. It binds to the growth hormone secretagogue receptor(GHSR) causing release of growth hormone from the pituitary gland.

Now, Aeterna Zentaris Inc. today announced that the Company has received a Complete Response Letter (“CRL”) from the U.S. Food and Drug Administration (“FDA”) for its New Drug Application (“NDA”) for Macrilen (macimorelin), a novel orally-active ghrelin agonist, for use in evaluating adult growth hormone deficiency (“AGHD”). Based on its review, the FDA has determined that the NDA cannot be approved in its present form.


BPA Exposure May Change Stem Cells, Lower Sperm Production

BPA (Bisphenol A) and other estrogenic compounds hamper development of the stem cells responsible for producing sperm in mice, which suggests such exposure could contribute to declining sperm counts in men, according to a new study.
The study, published online today in PLoS Genetics, is the first to suggest that low, brief exposures to bisphenol-A, or other estrogens such as those used in birth control but found as water contaminants, early in life can alter the stem cells responsible for producing sperm later in life.
Exposure to estrogens “is not simply affecting sperm being produced now, but impacting the stem cell population, and that will affect sperm produced throughout the lifetime,” said Patricia Hunt, a geneticist at Washington State University who led the study.
BPA is a ubiquitous chemical found in most people and used to make polycarbonate plastic and found in some food cans and paper receipts. People also are exposed to synthetic estrogens used in birth control as they are commonly found contaminating water, even after treatment. 
The U.S. Food and Drug Administration banned BPA from baby bottles in 2012 but maintains that BPA currently used in food containers and packaging is safe. And this week the European Food Safety Authority announced in a new assessment there is “no consumer health risk from bisphenol-A exposure.

Study indicates major added benefit of propranolol in some children with haemangioma

The Institute for Quality and Efficiency in Health Care (IQWiG) investigated in a dossier assessment whether propranolol offers an added benefit in comparison with the appropriate comparator therapy in infants with proliferating infantile haemangioma (sometimes called "strawberry mark").

According to the findings, there is an indication of major added benefit of propranolol (structure) in some children, i.e. those with haemangioma with a risk of permanent scars or disfigurement. In contrast, an added benefit is not proven for children with life- or function-threatening haemangioma, or with ulcerated haemangioma with pain or lack of response to simple wound care measures, because informative data are lacking.

Three-drug combination produces better results in multiple myeloma patients

In the treatment of multiple myeloma, the addition of carfilzomib to a currently accepted two-drug combination produced significantly better results than using the two drugs alone, according to a worldwide research team led by investigators from Mayo Clinic.
Their findings will be reported online Dec. 6 in the New England Journal of Medicine, and presented on Dec. 7 at the annual meeting of the American Society of Hematology (ASH), held in San Francisco.

Interim analysis of the ASPIRE clinical trial, which enrolled 792 patients with relapsed multiple myeloma from 20 countries, found an "unprecedented" prolongation of the time patients were free of disease progression, says the study's lead investigator, Keith Stewart, M.B., Ch.B, a Mayo Clinic oncologist in Arizona. "Patients taking three drugs -- carfilzomib, lenalidomide and dexamethasone -- stayed free of disease progression for 26 months on average," he says. "No one has reported anything like this before for relapsed multiple myeloma."

Researchers found that adding carfilzomib to standard treatment (lenalidomide and dexamethasone) resulted in 8.7 months of longer remission, almost 50 percent longer than the standard two-drug combination (26.3 months versus 17.6 months).

The number of patients who responded to treatment was also significantly improved by adding carfilzomib to standard treatment -- 87.4 percent versus 66.9 percent-- and more than three times more patients had no detectable disease after the three-drug treatment (31.8 percent versus 9.3 percent). Although results were preliminary, there was also a trend toward improved overall survival, Dr. Stewart says. "Importantly, patients on the three-drug cocktail also reported a better quality of life despite a higher intensity of treatment," he says.

These findings highlight increasing success in treating myeloma, the second most common blood cancer, says Dr. Stewart.

"Survival of multiple myeloma has almost doubled over the last decade, and the very positive outcomes from use of the three-drug combination will likely further improve outcomes," he says. "This is a nice story to tell."
Lenalidomide, a potent derivative of thalidomide, affects immune system function. Dexamethasone is a steroid drug. Carfilzomib is a proteasome inhibitor approved for use in 2012 by the U.S. Food and Drug Administration (FDA) for patients with advanced, end-stage multiple myeloma. The drug specifically targets regulation of the proteins that fuel growth of multiple myeloma.

Cimetidine drug could be one of many common over-the-counter medicines to treat cancer...

We know that, Cimetidine is a histamine H2-receptor antagonist that inhibits stomach acid production. It is largely used in the treatment of heartburn and peptic ulcers. It has been marketed by GlaxoSmithKline (which is selling the brand to Prestige Brands) under the trade name Tagamet (sometimes Tagamet HB or Tagamet HB200). Cimetidine was approved in the UK in 1976 and was approved in the US by the Food and Drug Administration for prescriptions starting January 1, 1979.
Now, it has been concluded that, a popular indigestion medication can increase survival in colorectal cancer, according to research published in ecancermedicalscience. But in fact, scientists have studied this for years - and a group of cancer advocates want to know why this research isn't more widely used.

Phase IIb trial shows platinum-resistant ovarian patients treated with PM1183 live longer




Zeltia announces today that its pharmaceutical division PharmaMar has data from a group of 33 randomized patients with platinum-resistant ovarian cancer demonstrating that PM1183 (see structure) shows a significantly superior median overall survival compared to topotecan. In this multicenter Phase IIb randomized trial, platinum-resistant ovarian patients treated with PM1183 lived longer, with a median overall survival of 18.1 months, compared to topotecan, which only achieves 8.5 months. PharmaMar will launch a pivotal Phase III trial to confirm the efficacy of PM1183 in a larger population, and that is expected to be the final step before registration of PM1183 for platinum-resistant ovarian cancer. "The pivotal trial planned for next year will hopefully confirm the effects in survival that we observe in these patients, who are in need of drugs that will reduce the risk of death and extend their lives, without posing major side effects", says Arturo Soto, Director of Clinical Development, PharmaMar.
The Phase IIb trial consisted of a first stage, single arm approach to assess efficacy of PM1183 in platinum-resistant ovarian cancer patients, followed by a second stage, in which patients were randomized to be treated with PM01183 or topotecan as control group.

Results presented at the American Society of Clinical Oncology (ASCO) earlier this year showed a significant improvement in the other secondary endpoint, progression-free survival, as well as in the overall response rate, which was the primary endpoint1. The results found now for the overall survival of patients with platinum-resistant ovarian cancer treated with PM1183 add to the clinical benefit, measured as 70% of overall responses and stable disease, previously observed. The manageable safety profile previously described for these patients has not changed.

Resveratrol in red wine inhibits formation of inflammatory factors that activate cardiovascular diseases

A natural substance present in red wine, resveratrol, inhibits the formation of inflammatory factors that trigger cardiovascular diseases. This has been established by a research team at the Department of Pharmacology of the University Medical Center of Johannes Gutenberg University of Mainz (JGU) working in collaboration with researchers of the Friedrich Schiller University in Jena and the University of Vienna. Their results have recently been published in the scientific journal Nucleic Acids Research.

Despite the fact that they eat more fatty foods, the French tend to less frequently develop cardiac diseases than Germans. This so-called French Paradox is attributed to the higher consumption of red wine in France and it has already been the subject of various studies in the past. A number of research projects have actually demonstrated that the natural product resveratrol, present in red wine, has a protective effect against cardiovascular diseases. But what exactly is the reason for this? It seems that at least part of the protective effect can be explained by the fact that resveratrol inhibits the formation of inflammatory factors, a conclusion reached by the research team of Junior Professor Andrea Pautz and Professor Hartmut Kleinert of the Mainz University Medical Center following collaboration in a joint project with Professor Oliver Werz of the Friedrich Schiller University in Jena and Professor Verena Dirsch of the University of Vienna. In fact, the researchers discovered that the natural substance binds to the regulator protein KSRP and activates it. KSRP reduces the stability of messenger RNA (mRNA) in connection with a number of inflammatory mediators and thus inhibits their synthesis.

Antacid medicines improve overall survival in patients with head and neck cancer

Patients with head and neck cancer who used antacid medicines to control acid reflux had better overall survival, according to a new study from the University of Michigan Comprehensive Cancer Center.

Reflux can be a common side effect of chemotherapy or radiation treatment for head and neck cancer. Doctors at the University of Michigan frequently prescribe two types of antacids - proton pump inhibitors or histamine 2 blockers - to help treat this side effect.

The researchers looked at 596 patients who were treated for head and neck cancer. More than two-thirds of the patients took one or both types of antacid medication after their diagnosis.

Patients who were taking antacids had significantly better overall survival than those who did not take them. Proton pump inhibitors, which include drugs such as Prilosec, Nexium and Prevacid, had the biggest effect: a 45 percent decreased risk of death, compared to patients who did not take antacids. Patients taking histamine 2 blockers, such as Tagamet, Zantac or Pepcid, saw a 33 percent decreased risk of death.

"We had suspicions that these medications somehow had a favorable impact on patient outcomes. This led us to review our large cohort of patients and screen them for common medications, focusing on antacids. In fact, our study did show that people taking antacids are doing better," says lead study author Silvana Papagerakis, M.D., Ph.D., research assistant professor of otolaryngology--head and neck surgery at the University of Michigan Medical School and an adjunct clinical assistant professor at the U-M School of Dentistry.

Results of the study are published in the December issue of Cancer Prevention Research.
The researchers are not clear why these medications affect the cancer, although they have begun additional work to understand the mechanisms involved.

New targeted therapy shows promise in patients with ALK-positive advanced NSCLC

An international study involving Manchester researchers has found that for previously untreated lung cancer patients with a particular genetic change, a new targeted therapy is better than standard chemotherapy.

Some patients with non-small cell lung cancer (NSCLC) have changes in the anaplastic lymphoma kinase (ALK) gene, which can drive the development of their cancer. A drug recently developed by Pfizer, crizotinib (see structure), targets ALK and is currently given to patients with ALK positive lung cancer when their cancer has worsened after initial chemotherapy. Now doctors have investigated the use of crizotinib in patients with ALK positive lung cancer who have not yet received any chemotherapy treatment.

Fructose more toxic than table sugar, mouse study suggests

When University of Utah biologists fed mice sugar in doses proportional to what many people eat, the fructose-glucose mixture found in high-fructose corn syrup was more toxic than sucrose or table sugar, reducing both the reproduction and lifespan of female rodents.

"This is the most robust study showing there is a difference between high-fructose corn syrup and table sugar at human-relevant doses," says biology professor Wayne Potts, senior author of a new study scheduled for publication in the March 2015 issue ofThe Journal of Nutrition.

The study found no differences in survival, reproduction or territoriality of male mice on the high-fructose and sucrose diets. The researchers say that may be because both sugars are equally toxic to male mice.

Both high-fructose corn syrup found in many processed foods and table sugar found in baked goods contain roughly equal amounts of fructose and glucose. But in corn syrup, they are separate molecules, called monosaccharides. In contrast, sucrose or table sugar is a disaccharide compound formed when fructose and glucose bond chemically.

Potts says the debate over the relative dangers of fructose and sucrose is important "because when the diabetes-obesity-metabolic syndrome epidemics started in the mid-1970s, they corresponded with both a general increase in consumption of added sugar and the switchover from sucrose being the main added sugar in the American diet to high-fructose corn syrup making up half our sugar intake."

James Ruff, the study's first author and a postdoctoral fellow in biology, says, "Our previous work and plenty of other studies have shown that added sugar in general is bad for your health. So first, reduce added sugar across the board. Then worry about the type of sugar, and decrease consumption of products with high-fructose corn syrup."

The new study is the latest in a series that used a new, sensitive toxicity test developed by Potts and colleagues. It allows house mice to compete in the seminatural environment of room-size "mouse barns." Previous mouse studies with the test found harmful effects of inbreeding, the antidepressant Paxil and, last year, an added-sugar diet with fructose and glucose in amounts proportional to a healthy human diet plus three cans of soda daily. These health effects had been missed by conventional tests.

Cholesterol Drug Vytorin Linked to Reduced Heart Attack Risk

We know that, Ezetimibe/simvastatin  is a drug combination used for the treatment of dyslipidemia. It is a combination of ezetimibe (known as Zetia in the United States and Ezetrol elsewhere) and the statin drug simvastatin (known as Zocor in the U.S.). The combination preparation is marketed by Merck & Co. under the trade names Vytorin and InegyEzetimibe reduces blood cholesterol by acting at the brush border of the small intestine and inhibiting the absorption of cholesterol, leading to a decrease in the delivery of intestinal cholesterol to the liver.
Simvastatin is an HMG-CoA reductase inhibitor or statin. It works by blocking an enzyme that is necessary for the body to make cholesterol.

FDA Approves 'Abuse-Resistant' Narcotic Painkiller

Seeking to make it tougher for people to misuse prescription painkillers, the U.S. Food and Drug Administration on Thursday approved a new hydrocodone tablet that's designed to help thwart abuse.
Hydrocodone -- best known by the brand name Vicodin (combination of Hydrocodone and Paracetamol) - is a powerful opioid painkiller that has been tied to a surge in dangerous addictions across the United States.
The FDA said that newly approved Hysingla ER (hydrocodone bitartrate) is an extended-release tablet to treat pain severe enough to require daily, round-the-clock, long-term opioid treatment that can't be eased by other pain medications.
The drug is not approved for "as-needed" pain relief, the agency said.
Hysingla has features that are expected to reduce, but not prevent, abuse of the drug. According to the FDA, the tablet is difficult to crush, break or dissolve, making it tougher for abusers to snort or inject it.

Treating Irregular Heartbeat With Digoxin May Come With Risks

The widely used heart drug digoxin is associated with increased risk of death and hospitalization among patients who have the heart rhythm disorder atrial fibrillation but no evidence of heart failure, a new study finds.
Atrial fibrillation is a common form of irregular heartbeat that has been linked to a rise in risk for stroke among older Americans. Digoxin has been used for more than a century to help treat irregular heartbeat, the authors of the new study said, and many guidelines recommend the drug for the treatment of atrial fibrillation.
However, the new findings "suggest that the use of digoxin should be re-evaluated for the treatment of atrial fibrillation in contemporary clinical practice," study co-author Dr. Anthony Steimle, chief of cardiology at Kaiser Permanente Santa Clara Medical Center, said in a Kaiser news release.
One expert wasn't surprised by the findings.

FDA Approves Olysio (simeprevir) in Combination with Sofosbuvir for Genotype 1 Chronic Hepatitis C Infection

Janssen Therapeutics, Division of Janssen Products, LP (Janssen) announced the U.S. Food and Drug Administration (FDA) has approved Olysio (simeprevir), a hepatitis C virus (HCV) NS3/4A protease inhibitor, in combination with sofosbuvir as an all-oral, interferon- and ribavirin-free treatment option for genotype 1 chronic hepatitis C (CHC) infection in adult patients as part of a combination antiviral treatment regimen. Sofosbuvir is an HCV nucleotide analog NS5B polymerase inhibitor developed by Gilead Sciences, Inc.

Basilea reports granting of U.S. orphan drug designation to isavuconazole for the treatment of invasive candidiasis

Isavuconazole (BAL4815) is an experimental triazole antifungal. Its prodrug, isavuconazonium sulfate (BAL8557) is currently in two Phase III clinical trials (SECURE and VITAL), the results of which are expected in the second half of 2013. 

Basilea Pharmaceutica Ltd. reports today that the U.S. Food and Drug Administration (FDA) has granted orphan drug designation to isavuconazole for the treatment of invasive candidiasis/candidemia, a potentially life-threatening infection caused by Candida yeasts. Isavuconazole has previously been granted orphan drug status in the European Union and the U.S. for the treatment of invasive aspergillosis and mucormycosis.

FDA Advisory Committee Recommends Savaysa (edoxaban) for Reduction of Embolic Events in Non-Valvular Atrial Fibrillation

Edoxaban (INN, codenamed DU-176b, trade name Lixiana) is an anticoagulant drug which acts as a direct factor Xa inhibitor. It is being developed by Daiichi Sankyo. It was approved in July 2011 in Japan for prevention of venous thromboembolisms (VTE) following lower-limb orthopedic surgery.

Daiichi Sankyo Company, Limited (hereafter, Daiichi Sankyo) today announced that the U.S. Food and Drug Administration’s (FDA) Cardiovascular and Renal Drugs Advisory Committee voted 9 to 1 to recommend approval of once-daily Savaysa (edoxaban) 60 mg dosing regimen for the reduction in risk of stroke and systemic embolic events (SEE) in patients with non-valvular atrial fibrillation (NVAF). Members of the committee also provided their opinions on the use of Savaysa.

FDA Approves sNDA for Invega Sustenna (paliperidone palmitate) for Schizoaffective Disorder

We know that, Paliperidone (trade name Invega), also known as 9-hydroxyrisperidone, is a dopamine antagonist and 5-HT2A antagonist of the atypical antipsychotic class of medications. It is developed by Janssen Pharmaceutica. Invega is an extended release formulation of paliperidone that uses the OROS extended release system to allow for once-daily dosing.
Paliperidone palmitate (trade name Invega Sustenna, named Xeplion in Europe and other countries) is a long-acting injectable formulation of paliperidone palmitoyl ester indicated for once-monthly injection after an initial titration period. Paliperidone is used to treat mania and at lower doses as maintenance for bipolar disorder. It is also used for schizophrenia and schizoaffective disorder.


Janssen Pharmaceuticals, Inc. today announced that the U.S. Food and Drug Administration (FDA) approved the supplemental New Drug Applications (sNDAs) for the once-monthly atypical long-acting antipsychotic Invega Sustenna (paliperidone palmitate) to treat schizoaffective disorder as either monotherapy or adjunctive therapy. The symptoms of schizoaffective disorder are complex and, without treatment, disabling. GABA injections prevent and reverse Type 1 diabetes in mice

A chemical produced in the pancreas that prevented and even reversed Type 1 diabetes in mice had the same effect on human beta cells transplanted into mice, new research has found. GABA, or gamma-aminobutryic acid, is an amino acid produced by the same beta cells that make and secrete insulin. 

Drs. Gerald Prud'homme and Qinghua Wang of the Keenan Research Centre for Biomedical Sciences of St. Michael's Hospital published a paper in 2011 showing for the first time that GABA injections not only prevented Type 1 diabetes in mice, but even reversed the disease.
A new paper published (Nov. 29) in the December issue of Diabetes shows GABA does the same thing in mice who have been injected with human pancreatic cells.

Type 1 diabetes, formerly known as juvenile diabetes, is characterized by the immune system's destruction of the beta cells in the pancreas. As a result, the body makes little or no insulin. The only conventional treatment for Type 1 diabetes is insulin injection, but insulin is not a cure as it does not prevent or reverse the loss of beta cells.

Drs. Prud'homme and Wang also found that GABA vastly improved the survival rate of pancreatic cells when they were being transplanted into mice. About 70 per cent of pancreatic cells die between the time the organ is harvested and transplanted. The researchers said their finding could lead to future research specifically related to pancreatic transplants.

Now Avanir Pharmaceuticals, Inc. (NASDAQ: AVNR) today announced that the U.S. Food and Drug Administration (FDA) has issued preliminary written feedback to its New Drug Application (NDA) for AVP-825. AVP-825 is a drug-device combination product consisting of low-dose sumatriptan powder, delivered intranasally utilizing a novel Breath Powered delivery technology. The FDA has raised questions regarding the human factor validation study data submitted as part of the NDA. Human factor testing focuses on the interactions between people and devices. The goal of human factor testing is to evaluate use-related risks and confirm that users can use the device safely and effectively. Although the NDA review is ongoing, Avanir has concluded, at this time, that approval of AVP-825 may be unlikely by the PDUFA date of November, 26, 2014

Experimental anti-cancer drugs PF-04691502 and PD-0325901 excel against colorectal cancer models




Genes make proteins and proteins tell your body's cells what to do: one talks to the next, which talks to the next, and to the next. Like a game of telephone, researchers call these  "signaling pathways". Abnormalities in these signaling pathways can cause the growth and survival of cancer cells. Commonly, mutations or rearrangements of genes in the MAPK  signaling pathway create cancer's fast growth, and alterations in the PI3K signaling pathway allow cancer cells to survive into virtual immortality.

Of course, researchers have extensively targeted these two signaling pathways, designing drugs that turn on or off genes in these pathways, thus interrupting the transmission of cancer-causing signals. Unfortunately, these pathways have proven difficult to drug and also it has been difficult to show the effectiveness of drugs that successfully interrupt the transmission of signals along these pathways.

A study by the University of Colorado Cancer Center published in the journal PLoS ONE and concurrent phase I clinical trial is examining a new strategy: targeting both these important cancer-causing pathways simultaneously.

"Well, these two pathways are mutated frequently in cancer. Why not hit both of them? It was as simplistic as that," says Todd Pitts, MS, research instructor in the Program for the Evaluation of Targeted Therapies, and the study's first author.

The study used colorectal cancer tumors grown on mice from samples of patient tumors, called "patient-derived xenograft" models. To these tumors, Pitts and colleagues added the experimental anti-cancer drugs PF-04691502 (left structure) and PD-0325901 (right structure), the first of which mutes a link in the PI3K signaling pathway and the second of which mutes a link in the MAPK signaling pathway. In this case, the combination was greater than the sum of the parts - alone, PF-04691502 and PD-0325901 modestly inhibit the growth and survival of colorectal cancer in these models; after 30-day exposure to the combination, colorectal cancer cells were killed much more effectively than by either drug alone, and even more effectively than if you added together the cells killed by each drug alone.

Scientists devise powerful algorithm to improve effectiveness of research technology harnessing RNAi

Scientists at Cold Spring Harbor Laboratory (CSHL) have devised a powerful algorithm that improves the effectiveness of an important research technology harnessing RNA interference, or RNAi.

Discovered in the late 1990s, RNAi is a naturally occurring biological mechanism in which short RNA molecules bind to and "interfere" with messages sent by genes that contain instructions for protein production. Such interference can prevent a gene from being expressed. In addition to helping regulate gene expression, the RNAi pathway in many species, including humans, acts to defend the genome from parasitic viruses and transposons.

Harnessed by scientists since the mid-2000s, RNAi has provided a way to artificially "knock down" the expression of specific genes. By preventing a gene or genes from being activated in a model organism such as a mouse, for instance, much can be learned by inference about gene function. RNAi-based technology also has been extremely useful as tool in drug discovery.

Researchers demonstrate efficacy of Bozepinib drug against cancerogenic stem cells





An Andalusian team of researchers led by the University of Granada has demonstrated the efficacy of a new drug against cancerogenic stem cells, which cause the onset and development of cancer, of relapse after chemotherapy and metastasis. This drug, called Bozepinib, has proved to be effective in tests with mice. The results have been published in the prestigious journal Oncotarget.

Cancerogenic stem cells appear in small quantities in tumours, and one of their important features is that they contribute to the formation of metastasis in different places within the original tumour. Cancerogenic stem cells remain dormant under normal conditions (i.e. they do not divide). Conventional chemotherapy and radiotherapy act upon those cancer cells which are clearly differentiated—i.e. which are undergoing processes of division—but they cannot destroy these dormant cancerogenic stem cells. Actually, after a positive initial response to treatment, many cancer patients suffer a relapse because these cancerogenic stem cells have not been destroyed.

Experimental drug works best when patients' immune cells surrounding tumors express PD-L1

A promising experimental immunotherapy drug works best in patients whose immune defenses initially rally to attack the cancer but then are stymied by a molecular brake that shuts down the response, according to a new study led by researchers at Dana-Farber Cancer Institute and the Yale University School of Medicine.

The antibody drug, known as MPDL3280A, inhibits the brake protein, PD-L1, reviving the response by immune killer T cells, which target and destroy the cancer cells. In recent clinical trials, the PD-L1 checkpoint blocker caused impressive shrinkage of kidney, melanoma, and lung tumors. But, as with other immunotherapy drugs, many patients saw no benefit.

Researchers report in the November 27 edition of Nature that the antibody was most effective when the patients' immune cells surrounding tumors expressed PD-L1 - a sign that a pre-existing immune response had been shut down by PD-L1. There was less tumor shrinkage in patients who never developed an immune response to the cancer - and, as a result, had less PD-L1 in the cancer and surrounding tissues.

"I think this is a launching point to use these findings as a predictive biomarker," said F. Stephen Hodi, MD, of Dana-Farber, senior author of the report. Hodi directs the Center for Immuno-Oncology and the Melanoma Treatment Center at Dana-Farber. First author is Roy Herbst, MD, PhD, chief of Medical Oncology at the Yale Comprehensive Cancer Center.

The scientists studied tumor tissue samples from 175 patients treated in clinical trials with MPDL3280A for advanced non-small cell lung cancer, melanoma, kidney cancer, and other cancers. On average, 18 percent of the patients had complete or partial shrinkage of their tumors, with higher or lower rates in different cancer types. Overall, the treatment was well-tolerated, with few severe side-effects, the report said.
